Who We Serve

To be eligible for Supported Living Services (SLS) through Plan of Care (POC), an individual must be at least 18 years of age, be clinically diagnosed with an intellectual and developmental disability (IDD) - as defined in Section 4512(a)(1) of the California Welfare and Institutions Code, and be referred to us by Alta California Regional Center (ACRC).

If you are an established client through a regional center and may be interested in receiving SLS through POC, please contact your regional center's Service Coordinator to initiate your referral through Alta California Regional Center (ACRC).
