Who We Are

Plan of Care (POC) is a Supported Living Services (SLS) agency committed to serving adult individuals with intellectual and developmental disabilities (I&DD). We advocate for their rights to choose how they want to live - and with whom. We support their choices, respect their values, and promote a culture of inclusion. Our services are personalized and may range from minimal assistance to 24-hour care.
