Our Services

Plan of Care (POC) provides its services through integration of two fundamental applications: 1) Person-Centered Care; and 2) The Principles of Supported Living Services (SLS).

Person-Centered Care

Each of our clients’ autonomy, participation, choices & preferences, dignity, respect, rights, individuality, and uniqueness are taken into full account from planning, execution, and the eventual achievement of their goals. Neither of our clients’ goals are standardized nor considered a one-size-fits-all.

The Five Principles of Supported Living Services (SLS)

In accordance with the California Department of Developmental Services (DDS) Section 4689(a) of the Lanterman Act, and Title 17 of the California Code of Regulations (CCR), the five Principles SLS are as outlined below.

1) A Home of One’s Own:

Individuals live in homes they own, lease, or rent, similar to other community members. They decide who lives with them, how their home is decorated, and are not forced to move if their service agency changes.

2) Choice and Self-Directed:

Individuals exercise control over their daily lives, including managing their own schedules, personal affairs, and choosing their own staff.

3) Relationships:

Services promote the development and maintenance of personal relationships, including family, friends, and neighbors, helping to build a natural, supportive social network.

4) Community Membership:

Individuals are supported to fully participate in their community, including employment, volunteering, and accessing local resources, fostering inclusion rather than isolation.

5) Flexible, Tailored Support:

Support is customized to the specific, changing needs of the individual, which may range from minimal assistance to 24-hour care.
